We are moving into Phase 1B and Phase 1C of Vaccine Distribution. Howard County Government has released information about the phased distribution of the Covid-19 vaccine. Here are links to the information.
IF YOU ARE AGE 75 OR OLDER, you are in Phase 1B and should go to this Howard County Health Department website and fill it in.

They will contact you with details after that. Certain other categories fall into this Phase 1B as well: those in assisted living, independent living, developmental disabilities/behavioral health group homes and other congregate facilities; high-risk incarcerated individuals, continuity of government, and education, including K-12 teachers, support staff, and child care providers.

They will contact you with details when Phase 1C begins in a couple of weeks. Other categories in Phase 1C are: workers in other critical sectors, including grocery stores, public transit, food/agriculture production, lab services, and manufacturing.
The “survey” is extremely simple. It basically consists of telling the Health Department your birthdate and how to reach you.
