Here are various forms that might be needed by a student, parent or adult participant in youth activities for the things we do. As forms and permission slips are needed and expire, they will be added or removed here.
Permission Slip
Permission slips for Student Ministry events that take place off church property are usually mailed home in the form of postcards. Here is a generic permission slip that you can use if you lose the postcard or want to invite friends.
